- Media Release Form signed and dated.
- Application for the one (1) category in which you are competing.
- Recent Photo (does not need to be professional) with application.
- Resume or Bio with application.
- 500 Word Essay with application on only one (1) of the following:

- How will the performing arts be a part of your future?
